How to Request a Michigan Immunization Record

If you are 18 years of age and older, you may be able to download your State of Michigan immunization record from the Michigan Immunization Portal using a government-issued photo ID. Check out our tutorials for ComputeriPhone, and Android for assistance.
To get a record for your child or dependent, or to get your own record without using the portal, contact your healthcare provider or child’s pediatrician or your county’s Local Health Department, or submit a Request Official State of Michigan Immunization Record Form.

Healthcare providers must report ALL immunizations administered to every child born after December 31, 1993, and under 20 years of age within 72 hours of administration.

If you are 18 years of age and older you may be able to download your State of Michigan immunization record from the Michigan Care Improvement Registry. A valid government-issued photo ID such as a state ID, driver’s license, or US passport. Visit the MI Immunization Portal: https://mdhhsmiimmsportal.state.mi.us/

Out-of-state IDs are accepted by the portal, but the address submitted to the portal must match the address in your MCIR record to download successfully. You can enter in previous Michigan addresses, or update your address using our Request to Change Information Form.

Request a State of Michigan Immunization Record for Adults, Children, or Legal Dependents

See below for additional options to obtain a copy of an Official State of Michigan Immunization Record if you cannot access the Michigan Immunization Portal or are looking for an immunization record for your child or dependent.

Healthcare
Provider

Request a copy of your or your child/dependent’s record from a healthcare provider such as your physician or child’s pediatrician.

Be sure to request the Official State of Michigan/MCIR record. Some doctor’s offices may provide a record from their electronic health record system, which may not be sufficient for travel or school requirements.

Local Health Department

Request a copy of your or your child/dependent’s record from your county’s Local Health Department.

If you no longer live in Michigan, contact the health department of the county you lived in previously.

If you have moved to Michigan, the health department can assist in entering immunizations from your previous state/country.

Request a copy of your or your child/dependent’s record using the Official State of Michigan Immunization Record Request Form.

Form requests are accepted via mail, fax, or email. Please note, it may take up to 2 weeks to process your request. International requests must include an email address. We cannot fax internationally.

History of the Michigan Care Improvement Registry

The Michigan Care Improvement Registry (MCIR) is a database that documents immunizations given to individuals in Michigan. MCIR was created in 1998 to collect reliable immunization information for children and make it accessible to authorized users. A 2006 change to the Michigan Public Health Code enabled MCIR to transition from a childhood immunization registry to a registry for all ages.

MCIR benefits healthcare organizations, schools, childcares, pharmacies, and those receiving immunization care in Michigan by combining immunization information from multiple providers into one immunization record. This reduces vaccine-preventable diseases and over-vaccination, allowing providers to view up-to-date patient immunization history in one system.

Your immunization record provides a history of all the vaccines reported to the Michigan Care Improvement Registry (MCIR). 

If a vaccine is missing from your immunization record, contact the provider who administered the vaccination to request they add it to your MCIR record.
